Major Finding

In patients who have respiratory failure post extubation noninvasive ventilation does not reduce reintubation rates and increases mortality.

Point for discussion:

If NIV is started earlier in those patients with respiratory failure post-extubation or selectively used in COPD patients, would re-intubation rates actually be reduced?
