Major Finding

The use of early non-invasive ventilation in patients at high risk of extubation failure can reduce the risk of respiratory failure (NNT 6) but has no effect on re-intubation rates, hospital mortality or ICU length of stay.

Point for discussion:

As 50% of the patients in this study had COPD, is this further evidence that we should have a low threshold to extubate these patients to NIV?
