Reference

Eichacker PQ, Gerstenberger EP, Banks SM, Cui X, Natanson C.
Meta-analysis of acute lung injury and acute respiratory distress syndrome trials testing low tidal volumes.
American Journal of Respiratory and Critical Care Medicine 2002 166:1510-4.

Major Finding

Ventilation of ARDS patients with low tidal volumes (5 to 7 ml/kg measured body weight) has not produced consistent beneficial effects in clinical trials of patients with ALI and ARDS. This analysis highlights three beneficial trials (where ventilation with low tidal volumes improved clinical outcome) and 2 non-beneficial trials where low tidal volumes were not associated with improved mortality.

Point for discussion:

Why does a low tidal volume strategy appear to work in some ARDS patients but not in others?
